An Office Visit with a Specialist

       A Happier, Healthier You

Before we reach out to you for scheduling, our rheumatologist thoroughly  reviews each new patient chart notes from your physician. We may see you for one visit in some cases and will provide assessments and advice to your primary care provider and other specialists.

  • Our MD may determine he is not the right specialist for you, and will inform your referring provider of this assessment.

  • FIRST VISIT with us  is simple.

  • We require a formal request from your physician.

  •  Ask them to send us your recent medical history, and their reason for your FIRST VISIT with a rheumatologist. We DO NOT accept various Insurance Plans  (click  here).

  • We require that our patients are either under the care of a primary care provider, or another specialist.

  • Fill out the required 'Record Release' at your referring provider  office, and they will send us your medical information. We have a Record Release you may also use.

  • Be certain your referring provider office has requested an 'insurance referral' from your insurance to our office, if necessary per your insurance contract.

We will contact you with several appointment options. If your physician requests an urgent FIRST VISIT, we will fit you into our schedule quickly.
